Is bone grafting always necessary in revision reverse total shoulder arthroplasty with uncontained glenoid bone defects?

Background Patients with an uncontained glenoid bone defect can still successfully undergo a reverse total shoulder arthroplasty (RTSA). Currently, there is tendency toward reconstruction of the premorbid plane grafts, which technically demanding. We investigated whether central peg positioning in spine pillar (CPPSP) more feasible alternative to use grafts. Methods This study included 60 revisions RTSA defects. were treated grafts 29 cases and CPPSP technique 31 cases. assessed clinical results using Constant score complication rate. Results The changed from 42 69 points group 47 graft group. difference increase was significant (P = .031) owing strength favor overall rate 37.7% (20 53 patients), reoperation 18.9% (10 53). Dislocations occurred only (n 3), loosening 3). Conclusion defects undergoing revision obtain similar compared valid but different complications.
